What To Include In Your Referral
To help us respond promptly and safely, please include where possible:
- Patient's name and contact details (with their consent)
- Reason for referral and the supports required
- Relevant clinical information, risks, or care considerations
- Funding source (NDIS, aged care, or private) if known
- Preferred start date or discharge timeframe
- Your details and preferred method of contact
