What To Include In Your Referral
To help us respond quickly and match the right support, it's helpful to share:
- Participant's name and preferred contact details (with their consent)
- NDIS number and plan management type
- Relevant plan goals and funded supports
- The supports you're seeking and preferred start date
- Any important information about communication needs, cultural preferences, or risks
- Your details as the referring Coordinator
How we work with you
Once we receive your referral, we'll confirm receipt promptly and let you know whether we have capacity to meet the participant's needs. We'll arrange an intake conversation, keep you informed throughout onboarding, and maintain clear, ongoing communication for the life of the support.
