What to Include in Your Referral
To help us respond quickly and match the right support, it's helpful to share:
- Participant name, date of birth and location
- NDIS plan details and funding type
- Goals and support needs
- Any specific preferences (gender, language, experience)
- Start date and hours required
- Your contact details for follow-up
How We Work With You
Once we receive your referral, we'll confirm receipt promptly and let you know whether we have capacity to meet the participant's needs. We'll arrange an intake conversation, keep you informed throughout onboarding, and maintain clear, ongoing communication for the life of the support.
